Field Care Manager (Hybrid/Remote) - Health Plan / Medical RN - Local to NY/NJ Border Region

Salary Range:

$90,000 - $120,000 annually

Company is seeking a dedicated Field Care Manager (FCM) to support our expanding Independent Practice Association (IPA) provider network across the New York metropolitan area. This hybrid field-based role is ideal for clinically experienced, bilingual healthcare professionals who excel at the intersection of patient care, provider engagement, and operational execution.

As a Field Care Manager, you will act as a trusted partner to provider offices, helping close care gaps, improve quality metrics, strengthen care management processes, and share best practices-especially among Korean-speaking patient populations.

This position requires routine in-office practice visits across New York and bordering areas of New Jersey, combined with remote administrative and coordination work.


Key Responsibilities

  • Serve as the primary liaison between Company/IPA leadership and assigned medical practices
  • Support providers and staff in closing care gaps, including Annual Wellness Visits, preventive screenings, and chronic condition management
  • Assist with care coordination activities and patient outreach
  • Share best practices on documentation, clinical workflows, and value-based quality metrics
  • Provide hands-on support to clinical and front-desk teams during onsite practice visits
  • Facilitate communication between practices and Company operations, quality, and data teams
  • Support rollout and adoption of value-based care initiatives and performance improvement programs
  • Monitor progress, identify challenges, and provide actionable feedback from the field
  • Participate in hybrid work activities including reporting, planning, and virtual meetings

Qualifications

  • Active RN license required
  • Clinical or medical office background strongly preferred (RN, MA, PA, or prior medical office experience)
  • Fluency in English and Korean required
  • Experience in primary care, care coordination, population health, or value-based care preferred
  • Comfortable engaging directly with physicians, clinical staff, and administrative teams
  • Strong interpersonal, communication, and organizational skills
  • Willingness to travel frequently between practices across the NY/NJ metro region
  • Self-driven, dependable, and able to work independently in a field-based environment
  • Familiar with EMRs and common productivity tools (Microsoft Office and/or Google Workspace)
